Step 1: Before you start the recredentialing application

Please note that this application has been pre-populated with information that we currently have on file for you. When viewing the application form you should:

  • Review any information already filled out on the application form.
  • Change any information that is not accurate or complete.
  • Enter additional information for all applicable areas.

Keep in mind:

  • previous work experience within the last 2 years, e.g.:
    • Kaiser Permanente
    • current and previous private/group practices
    • professional affiliations (hospital staff memberships and facility associations)
  • education and training within the last 2 years
  • current professional references who:
    • have medical practice licenses which are similar or higher than your license
    • have a similar practice as yours
    • have direct knowledge of your clinical competence, character, conduct and relationships with patients, staff and colleagues
  • current and prior professional liability insurance coverage within the last 2 years. 

  • NOTE:  Kaiser Permanente employed practitioners are afforded coverage through the Kaiser Permanente Medical Care Program and, as such, should list this as their Current Insurance Carrier under Section 7A of the application.

Please set aside at least 30 minutes to complete the online application. Once you start the application process, you will be able to save your entries and work on it at a later date until you submit the application.
