How recredentialing works

  1. You will complete your application online, then fax or email any required documents.

  2. CVS will verify all your credentials and send the completed credentials file to the applicable credentials and privileges facility(s) for review and action.
  3. If you are renewing participation with Kaiser Foundation Health Plan, Inc. (KFHP), your application will be evaluated by the appropriate Credentials and Privileges (C&P) Committee.  You will only receive notification if there is a change in your credentialing status.

  4. If you are renewing privileges at any Kaiser Foundation Hospital (KFH) and/or ambulatory surgical center, you will receive a seperate privileging application  from each of the KFH facilities where you need privileges. These documents should be returned to the appropriate Medical Staff Credentials Office for processing. Your recredentialing application and privileging application will be evaluated by the KFH Board of Directors.  You will receive a letter from each applicable hospital and Ambulatory Surgical Center notifying you of the Board’s decision. 

     

    Note: Privileges granted at one location are not transferrable to any other location. Each facility grants privileges seperately.
