Community benefit: serving and empowering the community
For nearly 60 years, Kaiser Permanente’s social mission has been the foundation of its community service programs. As a value-driven, not-for-profit integrated health care organization, Kaiser Permanente is dedicated to improving the health of our members and the communities we serve.
Through partnerships with community organizations and public entities, Kaiser Permanente serves the community by addressing issues and concerns that affect overall community health. Many of these partnerships are created and/or sponsored through grants that support community activities, projects and programs.
The Kaiser Permanente Santa Clara Service Area Grants Program responds to requests from organizations that provide service throughout Santa Clara Area communities: Santa Clara, Milpitas, Campbell, Mountain View, North San Jose, West San Jose, Downtown San Jose, Cupertino, Saratoga, Los Gatos, Los Altos and Campbell.
Community benefit grants program
Every three years, Kaiser Permanente undertakes a strategic planning process with the Community Benefit Coalition of Santa Clara County consisting of diverse public and private stakeholders. It collects and analyzes important health data and trends to assess community health needs in order to collaboratively develop a comprehensive plan to help improve the health of our communities. The Grants Program provides the opportunity to partner with locally focused organizations that provide direct services or impact public policy related to improving the health and well being of our communities.
Kaiser Permanente Santa Clara Service Area’s Priority Health Goals 2008─2010
- To improve and promote healthy lifestyles for children and youth.
Programs need to aim at improving and promoting healthy lifestyles for children and youth, with an emphasis on obesity, healthy eating and active living programming via family oriented strategies, systems approach and collaboration. Proposals with the goals of serving the underserved and vulnerable children and youth by providing health education and prevention programs to youth who are engaging high risk activities will still be considered. - To increase access to health care to the underserved – due to income, language, culture or age barriers.
Programs need to aim at improving access to health care services for the underserved populations with emphasis on older adults, persons with chronic diseases or linkage/establishment of "medical homes."

Process of Kaiser Permanente Santa Clara Service Area Grants Application
Cycle 1:
- Please submit the completed proposal and required supplemental attachments.
- Please see attached Grant Checklist for important deadlines
- Please be advised that one original and three copies of the proposal need to be submitted at the address below by noon of Thursday, March 27, 2008. Also please email your proposal and budget document for expeditious confirmation and processing.
- Kaiser Permanente will contact you to notify you whether or not your organization’s proposal has been funded by June13, 2008.
Cycle 2: Please submit a Letter of Inquiry by August 31, 2009.

Grant guidelines
Kaiser Permanente grants are limited to organizations that are exempt from taxation under section 501 of the Internal Revenue Code.
Before completing the Grants Application Form, please take a moment to read the guidelines provided below:
Kaiser Permanente's Grants Program does not fund:
Religious (for religious purposes), partisan political, fraternal, athletic, international or social organizations, endowments, memorials or individuals, field trips or tours. Fundraising events such as raffles, telethons, walkathons and auctions are also ineligible.
Funding must be used for the program for which funds were requested.
Agencies who previously received Kaiser Permanente grant funds are eligible to reapply each year and will be considered along with all other qualifying proposals.
Funding decisions are made based on recommendations from the grants review committee and availability of resources.
