Health Counseling by a Clinical Health Educator (CHE)
The CHE supports Adult Primary Care clinicians through provision of individual appointments with members referred for life style change and education. A CHE is an expert in:
- Educating patients on self-care
- Making lifestyle changes to enhance health and prevent disease complications
- Coaching on how to adhere with treatment plan
- Building confidence in member self care skills
- Facilitating group programs for patient support
Specific competency areas include: Cholesterol, Diabetes, HIV Test Counseling, Hypertension, General Nutrition Education, Physical Activity, Tobacco Cessation, Weight Management, and Advance Directive for Health Care
Internal Appointments Available
Monday – Friday 9 a.m. to 4:30 p.m.
