The Succeed™ program will help you examine what's affecting your overall health—from how often you exercise to what you eat in the morning.

After answering some questions about your overall health, you will receive a customized action plan to help you succeed in making healthier lifestyle choices. You will also receive follow-up e-mails to monitor your progress.

You can also choose to place a summary of your results in your electronic medical record. Your doctor will then have access to the summary and can work with you to achieve your health goals.
