Applying for Health Education Materials, Speakers and In-kind Donations

 

Organizations may also apply for in-kind services such as health education materials, equipment donations or guest speakers. These requests do not require an application however the following documentation is required before a request can be considered:

  • A request letter on the organization’s letterhead (not to exceed 2-pages)

  • A copy of current IRS determination letter indicating 501 c (3) tax-exempt status. 

  • The current Board of Directors list

Requests must be submitted at least 45 days in advance. Depending on the request, your letter should include the following information:

  • Description of the organization, its mission and values, area of focus, services provided and target population

  • Description of the request, including dates, duration of the activity, and location

  • Description of the donation being requested, and a brief explanation on how the equipment, supplies, and/or furniture will be used

These requests are accepted on an ongoing basis beginning
January 1, 2011 through October 31, 2011.  We can only give donations to qualified organizations located in East Bay. Requests for speakers are considered on a case by case basis.
