Good health is a fresh start.
At Kaiser Permanente Fresno, we strive to make it easy to get the care you need. Once your health coverage begins, follow these simple steps to start making the most of your new membership. If you need help, call our Member Service Call Center at 1-800-464-4000, weekdays from 7 a.m. to 7 p.m. and weekends from 7 a.m. to 3 p.m. For TTY for the deaf, hard-of-hearing or speech-impaired, call 1-800-777-1370.
Step 1
Get your medical record number.
As a new member, you’ll soon get your Kaiser Permanente ID card and membership information in the mail. Your ID card contains your unique medical record number. You’ll need your medical record number to choose your personal physician, make an appointment, and get care at our facilities. If you need care after your coverage begins but before you’ve received your ID card, call our Member Service Call Center at 1-800-464-4000.
Step 2
Choose your personal physician.
Visit kp.org/chooseyourdoctor to learn more about our physicians, where they work, and how to choose one who’s right for you. If you don’t have Internet access, refer to Your Guidebook to Kaiser Permanente Services to find the personal physician selection phone number for the facility of your choice. The Fresno Area phone number for personal physician selection is 559-448-4364.
Step 3
Register on our website, kp.org.
As a registered member on kp.org, you get free, secure access to My Health Manager, 24 hours a day, seven days a week. Go to kp.org/myhealthmanager to find the tools you need to take care of many of your health matters online. You can email your doctor’s office, schedule routine appointments, order prescription refills, view most lab test results and more.*
Step 4
Make your first appointment.
Schedule an appointment with your personal physician at kp.org/myhealthmanager.* If you don’t have Internet access, call our Member Service Call Center, then choose option 1 when prompted, and one of our representatives will schedule an appointment for you. If you haven’t received your ID card before your first appointment, please bring a copy of your enrollment form with you.
Step 5
Transfer your medical information.
To make your transition to Kaiser Permanente a smooth one, we recommend transferring your medical records and prescriptions from your previous health care provider. For more information about transferring your records, call our Member Service Call Center at 1-800-464-4000 or talk to your personal physician at your first appointment.
*To use the secure features of My Health Manager, you’ll need to register at kp.org/register, then sign up to tour our website with your user ID and password.
