Asthma & COPD Resources

Our Health Education Centers offer videos/DVDs (in English and Spanish), books, pamphlets, and Internet access to help you learn more about controlling your asthma or living with COPD. Call or visit to learn more.  You can also watch videos about Asthma and COPD by visiting kp.org/watch

 

 

Blood Pressure Screening

Volunteers offer free blood pressure checks on a drop in basis.  Open to the community
Location 1: Fremont Center, Niles building, volunteer desk 2nd floor.

Understanding Your High Blood Pressure

Anyone can have high blood pressure (also known as hypertension). Join us to learn the basics about high blood pressure,  self-care skills, and positive lifestyle changes that can help keep your heart healthy. Members only.

Cholesterol and Your Heart

Want to lower your cholesterol, but don’t know where to begin? This class helps you understand how exercise and a heart-healthy diet can lower your cholesterol, improve your overall health, and reduce your risk for heart attack and stroke. Members only.

Complex Chronic Conditions Case Manament Program

If you have more than one ongoing health condition, this program can help you develop a plan to manage your health, feel better, and stay as active as possible. A case manager will work with you and your physician to support you in reaching your goals.  Members only.

Healthier Living with Ongoing Health Conditions

Living with a chronic condition like diabetes or arthritis can sometimes be hard. In this class, you’ll learn skills to manage your physical and emotional health in order to live a fuller life. Open to the community.

Irritable Bowel Syndrome Overview

Find out more about Irritable Bowel Syndrome (IBS) and how to manage it in this two-hour overview, led by specialists in gastroenterology and behavioral medicine. Members only.

Multifit

MultiFit is for members who have recently had a heart attack, angioplasty, or bypass surgery. In this unique program,
a nurse works closely with you and your physician to design a personalized plan to help reduce your risk of further heart problems. To participate in this class, please talk to your doctor about a referral. Members only.
Location: Hayward Medical center, Cardiology Station.

Mended Hearts, INC

If you or a loved one has had heart surgery or is living with a heart condition, this group can provide education and opportunities to help you support one another. Open to the community.
Location: Hayward Medical center.

PHASE: Preventing Heart Attacks and Strokes Everyday

Find out what you can do to help prevent heart attacks and strokes through nutrition, exercise, stress management, medication, and lifestyle changes. We recommend this singlesession class for anyone living with heart disease, diabetes, peripheral arterial disease, or chronic kidney disease. Members only.

Pain Management Program

If you have been living with longstanding physical pain, this program— led by a team of multidisciplinary chronic pain specialists—can help you develop skills to manage your pain and take your life back. By referral only. Members only.

 

Parkinson's and the Art of Moving

If you live with Parkinson’s disease, this gentle movement and voice program can help you improve balance and coordination, reduce injuries from falls, and improve your speech. Special exercises to improve grip and dexterity, reduce tremor, and counteract the Parkinson’s mask are included. Open to the community.

Palliative Care
If The Paaliative Care Team can meet with you to help you understand you illness and your options, support you and your famly, and address your quiality of life.  We can assist you in clarifying your health care goals and developing a plan for your health care choices.  These services are provided both in the hospital and in outpatient clinic setting.  Members only.
Location: Hayward
