Complex Chronic Conditions Case Management Program

The Complex Chronic Conditions Case Management Program is a special Kaiser Permanente program designed to provide members who have more than one ongoing health condition the highest quality care.  This one to three-month program will help you manage your ongoing health conditions so that you can be as healthy and active as possible.  Any of your Kaiser Permanente health care practitioners can refer you to the program or you may refer yourself. The referral will be screened by the Complex Chronic Conditions team. I f the program is right for you, you will be contacted to schedule an intake interview. A specially trained nurse and social worker will work with you and with your primary care physician to develop a plan to help you manage your health care. Y our case manager will offer you help and support, but you will need to actively work on changes that you want to make; this program works best when you take an active part in developing and implementing your personal care plan. Healthier Living: Managing Ongoing Health Conditions

Living with an ongoing health condition such as diabetes or arthritis doesn’t mean you have to let it rule your life. Learn to manage your symptoms, fight fatigue and frustration, make daily tasks easier, and get on with your life. Open to the community.

Introduction to Irritable Bowel Syndrome Overview

Find out more about Irritable Bowel Syndrome (IBS) and start feeling better. In this 2-hour overview, specialists in gastroenterology and behavioral medicine explain what IBS is and how to manage it. Open to Kaiser Permanente members only.

Dates/Time: Third Tuesday of each month; alternating between 10 a.m.–noon and 3–5 p.m.

Kidney Awareness and Prevention of Chronic Kidney Disease

If you have early signs of mild kidney problems, find out what you can do to help slow or prevent progressive kidney damage that can result from diabetes, hypertension, and various kidney conditions. Open to Kaiser Permanente members only.
Location: Fremont and Hayward Medical Centers

Pain Management Program

Have you been living with longstanding physical pain that has not responded to previous interventions? If so, we can help you develop the skills to take your life back. Our Pain Management Program includes a multidisciplinary team of chronic pain specialists from internal medicine, neurology, physical medicine, psychology, physical therapy, and nursing. By referral only — call your primary care practitioner to determine if this program is right for you. Open to Kaiser Permanente members only.

 

Parkinson's and the Art of Moving: A gentle movement and voice exercise program

If you live with Parkinson’s disease, this gentle movement and voice program can help. Classes focus on stretching, balance, coordination, and reducing injuries from falls, as well as vocal exercises and speech training. Special exercises to improve grip and dexterity, reduce tremor, and counteract the Parkinson’s mask are included.  Open to the community
