Getting Started
Thank you for choosing Kaiser Permanente as your Health Care Provider. The below information is designed to help make your transition into our health care system as easy as possible:
Kaiser Permanente services at the Hayward/Fremont Medical Center
As a member, you can access services at any of our locations as soon as your coverage becomes effective. If you aren't sure of your exact effective date, ask your employer or call Member Services, 7 days a week 24 hours a day at 800-464-4000; TTY for the hearing and speech-impaired, 800-777-1370.
Your Kaiser Permanente identification card and informational material will be sent when your coverage becomes effective. However, since you may need health services as soon as your coverage is effective, the following information can help you get started.
Obtaining your medical record number
Printed on your Kaiser Permanente ID card will be your Medical Record Number. This card will be mailed to you by Member Services. The medical card number will be the point of reference for all care provided to you.
If your health coverage is effective and you need care before your card arrives, you can request your Medical Record Number. Call Member Services, 7 days a week 24 hours a day at 800-464-4000; TTY for the hearing and speech-impaired, 800-777-1370.
Choose your doctor
To choose a personal physician or nurse practitioner after you've obtained a Medical Record Number, simply go to Choose My Doctor Online or call Member Outreach at (510) 675-2663.

Transfer your existing prescriptions
If you have a prescription that was originally filled at a non-Kaiser Permanente pharmacy, you may request a transfer of the prescription to any of our pharmacies. Just call our Pharmacy Call Center at 1-877-795-2473, weekdays from 8 a.m. to 8 p.m. and Saturdays from 8 a.m. to 6 p.m.
When requesting a transfer, please be ready to provide your name, medical record number, prescription number, the previous pharmacy's name and phone number, and the Kaiser Permanente pharmacy where you would like to pick up your prescription. Our pharmacy team will handle the rest. Please note that transfers may take up to 48 hours.

Transfer your medical records
When you visit your personal physician for the first time, speak to him or her about your health history and goals. Your provider can determine what specific medical information needs to be transferred from your previous health care provider.
You will need to contact your previous provider and request that they forward your medical records to Kaiser Permanente.

Help in your language
Kaiser Permanente is committed to making your health care experience simple, useful, and caring. Our Fremont / Hayward / Union City Medical Centers have dedicated professional interpreters on staff for your medical appointments.
The languages offered by staff interpreters are Spanish, Chinese and American Sign Language (ASL). If you require another language, we use agencies that provide interpreters in those languages. We also offer phone interpretation when needed. If you would like to have an interpreter at your next appointment, please ask when you making your appointment.
What kp.org can do for you
Registering is simple and only takes a few minutes. In order to make sure your personal health information is secure, we'll need to ask you a few questions to verify your identity. After you answer the questions, you can choose a password* and start accessing the online features immediately. If you can't or don't wish to complete the process online, you can do so by mail.

When you register on kp.org, you'll get access to My Health Manager — the feature that enables you to take care of your health and view parts of your health record, all in one place.**
Once you register, you can:
• E-mail your doctor's office
• View select test results
• Order prescription refills (and have them mailed to you, with free shipping)
• Request or cancel routine appointments
• Review recent past office visits
• See a list of your recent immunizations and allergies
• Act for a family member (e-mail your child's doctor, and more)
*KP Members between the ages of 13 through 17 must receive their temporary password by U.S. mail.
**Some features are not yet available in all areas.
