How to Choose Your New Doctor
Welcome! We're delighted that you've chosen Kaiser Permanente as your partner in health. We look forward to a long and healthful relationship with you.
Here you will find information that is useful to new members. In addition, you can find links to many of our services by clicking new member.
We believe in personalized care and recommend that you select your personal doctor or nurse practitioner at the medical facility that you'll use most often. To choose your doctor or nurse practitioner, simply click Choose Your Doctor. If you prefer, call Member Outreach at 1-877-988-7868, Monday through Friday, 8 a.m. to 5 p.m.
- For MEN, please select a personal doctor or nurse practitioner from our Adult Medicine Department.
For WOMEN, please select a personal doctor or nurse practitioner from both our Adult Medicine and Women's Health Departments.
And of course, if you have CHILDREN, please select a doctor or nurse practitioner for your children from our Pediatrics Department.
Some of our facilities currently have Family Practice Medicine physicians who can take care of the needs of your entire family. Please contact Member Outreach at 1-877-988-7868 to find out more about this option.
Your Medical Record Number - Your Passport to Kaiser Permanente
Your medical record number is your passport to Kaiser Permanente and you will need it when you connect with us in person, online or over the phone. If you have not yet received your Kaiser Permanente identification card in the mail or if you misplaced it, you can request your medical record number by calling Member Services at 1-800-464-4000 or 1-800-777-1370 (TTY for the hearing/speech impaired) from 7 a.m. to 7 p.m. Monday through Friday, and 7 a.m. to 3 p.m. Saturday and Sunday.
