Thank you for choosing Kaiser Permanente as your health care provider. You can access services at any of our locations as soon as your coverage becomes effective. If you aren't sure of your exact effective date, ask your employer. You will receive your Kaiser Permanente identification card and medical record number in the mail after your coverage becomes effective. Please follow these steps to help you get started:

To choose a personal physician or nurse practitioner after you've obtained a Medical Record Number, simply visit “Choose a Personal Physician” on kp.org or call Member Services at 1-800-464-4000; TTY for the hearing and speech impaired, 1-800-777-1370.
With your Kaiser Permanente identification card handy, go to kp.org/register to set up your user ID and password. Once you've registered, you'll have free, around-the-clock access to the time-saving features of My Health Manager.
When you visit your personal physician for the first time, talk to him or her about your health history and goals. Your doctor can determine what medical information may need to be transferred from your previous health care provider.
New members should receive a medical record number from Kaiser Permanente before completing an Authorization for Disclosure Form and transferring medical records. Please make sure you have your Kaiser Permanente Medical Record Number on the form before sending materials.
Download and complete an authorization for disclosure form here, and take it to your non-Kaiser Permanente physician. We recommend that you keep a copy of your important medical information at home.
When you visit your Kaiser Permanente personal physician for the first time bring a copy of your medical information. Talk to your doctor about your health history. Your doctor can determine what medical information needs to be included in you Kaiser Permanente medical record.
As a member, you may transfer a prescription from a non-Kaiser Permanente pharmacy to any of our pharmacies. Be ready to tell the pharmacist your prescription number and the name and phone number of the outside pharmacy. Our Kaiser Permanente pharmacy team will handle the rest.
This transfer can take up to 48 hours. To use this service, simply call your Kaiser Permanente pharmacy toll-free at 1-877-RXKAISER (1-877-795-2473).
We have teams of physicians, registered nurses, dietitians, pharmacists, and other health care practitioners who specialize in providing care to our Spanish- and Chinese-speaking members.
