Heart Smart: Prevent Heart Attacks and Strokes Everyday

Learn how to Prevent Heart Attacks and Strokes Every Day (PHASE) through healthy lifestyles and managing medications. This class is for anyone who has diabetes, pre-diabetes, heart disease, chronic kidney disease, high blood pressure, high cholesterol, or has ever had a stroke or transient ischemic attack (TIA). Topics include medications, healthy eating, physical activity, weight management, and resources at our San Jose Medical Center. Taught by a clinical health educator. Open to Kaiser Permanente members only.

Take Heart

This class is designed to help heart patients and their families establish a healthy way of life. Topics include physiology, risk factors, medications, cholesterol management, exercise, and stress management. We recommend this series for patients who have experienced angioplasty, angina, heart attack, and coronary artery surgery. Patient support welcome to attend. Open to Kaiser Permanente members only.

Want to lower your cholesterol, but don’t know where to begin? This class helps you understand how exercise and a heart-healthy diet can lower your cholesterol, improve your overall health, and reduce your risk for heart attacks and strokes.
