Our Santa Clara Arques Medical Offices are scheduled to open on June 4. The new facility is at 1263 E. Arques Ave. in Sunnyvale. It is less than four miles from our Santa Clara Medical Center.
When the Arques location opens, our Santa Clara Kaiser Drive Medical Offices will close. You’ll find the following departments at our new facility, which are by physician referral:

Chronic Conditions Management (moving from Santa Clara Medical Offices)
Memory Clinic (moving from Santa Clara Medical Center)
Pain Management Rehabilitation Program/Acupuncture
Physical Medicine and Rehabilitation
