We recommend that you discuss these health issues with your doctor before proceeding with this program.

Once you are cleared by your doctor to do this program, we hope it helps you find relief from your back pain.

Get a Portable Electronic Medical Record

Get a portable electronic medical record (PEMR) for yourself or your family members to use when:

  • Children are away at camp or college.
  • You are traveling or vacationing outside of a Kaiser Permanente service area.
  • Your family members move to another state.

The PEMR contains selected important information from the electronic medical record, including:

  • Contact information and demographics
  • Personal physician(s)
  • Allergies
  • Immunization records
  • Medications prescribed or refilled in the past year
  • Lab results from the past year
  • Problem lists
  • Clinic visit information with diagnosis and procedures in the past year
  • Hospitalizations and emergency room visits in the past five years
  • Copy of Advanced Health Care Directive and Physician Orders for Life-Sustaining Treatment (POLST)
  • Most recent electrocardiogram (EKG) and chest x-ray

The information on the PEMR is created by doctors, for doctors, so if you view your information, some entries may not make sense to you. It is also not a complete copy of the medical record but will have important medical information that a new doctor can access quickly.

To obtain a PEMR flash drive for yourself or a family member, go to the Medical Secretary or Release of Information Department in the medical center nearest you. The first flash drive is available for a small fee, and updates are free.

Tips on viewing information on your flash drive

  • Attach your flash drive to one of the USB ports on a computer. The flash drive will be called KP-USB. When the file starts to open, you must enter your password to see its content.
  • To safeguard your privacy, all personal information on the flash drive is password protected. Kaiser Permanente does not keep a record of the flash drive password, so please keep a record of your personal password.
  • Behavioral Medicine and Chemical Dependency Recovery Program (CDRP) records are not included. However, if you are being treated for, or have discussed behavioral medicine/chemical dependency issues with your doctor, that information may be included in the summary.
  • Call your primary care doctor if you have questions about specific entries on your flash drive.
  • The content on your flash drive may become out of date. This can happen, for example, when there are changes in your health condition, prescriptions, immunizations, or other aspects of your care. For a new, current PEMR, contact the office where you originally obtained your flash drive.
