Before Your Visit

What to bring:

  • Pack your medications. It is very helpful to have the actual tubes or jars of medication that you are using with you at your visit.
  • Please bring your Kaiser Permanente medical card and a photo ID, such as your driver's license, with you.

Think about the questions and issues you would like to discuss with me.

  • It is a good idea to write them down and bring them to our visit.

During Your Visit

Before you see me, you will meet with my medical assistant (MA):

  • My MA will call you in from the waiting room and bring you to the exam room. You will also be asked if you smoke. If you do smoke and are ready to try to quit, my MA will give you information about the many services we provide to help you quit.

All of our exam rooms have computers with access to your medical record:

  • We can review your lab results, previous visits and pharmacy records during your visit. If you need a new prescription, I will sent it electronically to the pharmacy. You do not need a paper prescription.
Ask questions:
  • It is always good to ask questions and voice any concerns you may have about instructions, medical tests, medications, or treatments. If you don't understand my answers, please ask for more information.

Before you leave, my MA may give you a copy of your "After Visit Summary."

  • This summary includes my name and the date and time of our visit, my test orders, and your medications. It usually includes the instructions I've given you during our visit and any follow-up information.
  • You can refer back to your After Visit Summary if you forget what we discussed. You can also view your After Visit Summary online.

After Your Visit

If you have questions or are unsure about the plan we discussed at your visit:

  • You can view your After Visit Summary online. To view your After Visit Summary: click the "Past visits" link on the left hand side of this web page, under the heading "View information". Sign on to kp.org, and then click "Continue to view past visits". Select the After Visit Summary you would like to view by clicking on the Date/Time link for that visit.
  • You are also welcome to e-mail me with questions at anytime.

If I referred you for a consultation with a specialist:

  • And scheduled your appointment at the time of your visit, you will receive an appointment reminder in the mail.
  • And did not schedule an appointment during your visit, you will be contacted by the specialty department to arrange an appointment.
  • If we talked with a specialist by phone, we will also decide if you should follow up with the specialist or me if you have additional issues or questions.

If you had lab work or other tests done:

  • I'll contact you with your results by letter, e-mail, or phone.
  • You may view your results online. They are often available online before I contact you, so my comments and recommendations usually follow in a separate e-mail.
  • Biopsy results are not available online.

You can contact me:

  • By e-mailing me at any time for nonurgent questions.
