Help with Questionnaire

Why do I have to fill out questionnaires? 

Questionnaires can be assigned to you or your family member for various reasons. They may be used to help prepare for an upcoming visit or procedure. A doctor may use a questionnaire to help manage your or your family member's health. Or you may be given a questionnaire series to track progress during treatment or recovery after a procedure. Because questionnaires help us gather information needed to provide you or your family member with better care, we ask you to complete questionnaires assigned to you. 


What if I’m not able to complete a questionnaire? 

Please contact the doctor's medical office and talk about your concerns. 


I had a set of questionnaires I was supposed to take, but I clicked "opt out" and now I can’t find them. What do I do? 

Unfortunately, there isn't a way to "opt in" to a questionnaire once you've selected the "opt out" option. Please contact the doctor's medical office and ask if the questionnaire series can be sent to you again.
