Additional Information

Your Personal Physician

We request that each member select a personal physician in our department. Your personal physician will see you for both routine appointments and urgent problems and will be available to communicate with you regarding non-urgent issues by phone or by e-mail through SECURE MESSAGING.

Should your personal physician not be available, you may be scheduled to see another physician in the group or another member or our medical staff.

If you do not have a personal physician or wish to change your personal physician, you may click this link to CHOOSE A HEALTH CARE PROVIDER.
