My Choices: Next Steps Life Care Planning


When you have a serious illness, such as cancer, heart failure, COPD (chronic obstructive pulmonary disease), or kidney disease, you face many health care decisions.

Some of the most important decisions you make are the type of medical treatments you want or don’t want if you have a severe complication from your illness.

The best time to think about your treatment options is before a serious complication happens. 

If you or a loved one is living with a serious illness, we encourage you to participate in our Life Care Planning service called My Choices: Next Steps. During a My Choices conversation, we talk with you and your chosen health care agent about your illness and help identify your priorities for living well.

My Choices Conversation

During a Next Steps Life Care Planning conversation, we encourage you to think about your health care. You will discuss what type of treatment you want if you have a serious event, such as a heart attack or stroke.  

It is important for your health care agent to join you for these conversations. This way, your agent can confidently communicate on your behalf if you have a medical emergency in the future and are unable to speak for yourself.   

You may feel relieved to have an open, caring conversation that allows you to explore your personal beliefs and create goals for future care. 

What to Expect

During a Next Steps conversation, you, your chosen agent, and a specially trained Kaiser Permanente provider (facilitator) discuss your wishes.

Together we discuss:

  • What you know about your illness and its possible complications.
  • What living well means to you.
  • Any fears you have.
  • Your hopes for your medical care.

We offer you support to help you make informed choices about the type of medical care you do and do not want if you develop a serious complication.

Once we know your wishes, we will make sure your decisions are placed in your medical record.

In addition, we encourage you to talk about your wishes and choices with your health care agent, loved ones, and clinicians.

Your Next Step

If you are interested in My Choices: Next Steps, talk with your doctor or care manager. 

When appropriate, one of our facilitators arranges to meet with you and your health care agent. Often facilitators are social workers or nurses.  The My Choices: Next Steps appointment generally lasts about 90 minutes.
