Step 2: Before you submit the credentialing application

Please confirm that

• you have an active California license (or your application has been submitted)
• you have a DEA (if applicable) that reflects a California practice address
• all pages are complete
  • all applicable 'Does not apply' boxes are checked
  • all Yes/No questions are answered, including sub-questions
  • explanations are provided where required (malpractice cases, time gaps, criminal convictions, etc.)
  • date ranges are entered in the correct format , e.g., MM/YYYY
• social security number is provided
• time gaps greater than three months since graduation from medical school are explained in writing

While completing your application

• Please Save & Close your application periodically while you are working on it to prevent data loss.

• If you need application specific help, please refer to 'Help using application program' in the navigation bar or contact us.

• Please Save & Close your application prior to submitting it to ensure all your changes have been saved.  You will need to open your application to submit it.
