After you enter into a contractual or employment relationship with Kaiser Permanente, the Credentials Verification Service (CVS) will send you a password and link to this Web Site so you can complete a credentialing application.
You will complete your application online, then transfer any documents.
CVS will verify all your credentials and send the completed credentials file to the applicable credentials and privileges Facility(s) for review and action.
If you are applying for participation with Kaiser Foundation Health Plan, Inc. (KFHP), your application will be evaluated by the appropriate Credentials and Privileges (C&P) Committee. You will receive a letter from the Committee notifying you of their decision. You may not treat Kaiser Permanente members until your credentialing has been approved by the C&P Committee.
If you need privileges at any Kaiser Foundation Hospital (KFH) and/or ambulatory surgical center, you will receive a seperate privileging application from each of the KFH facilities where you need privilleges. These documents should be returned to the appropriate Medical Staff Credentials Office for processing. Your credentialing application and privileging application will be evaluated by the KFH Board of Directors. You will receive a letter from each applicable hospital and Ambulatory Surgical Center notifying you of the Board's decision. If temporary privileges are granted prior to the Board’s approval, you will also receive notification. You may not provide services at any KFH hospital or ambulatory surgical center until appointment and/or privileges have been granted by that location.
NOTE: Privileges granted at one location are not transferrable to any other location. Each facility grants privileges seperately.
If you are applying for employment with The Permanente Medical Group, Inc., passing credentialing is a requirement for employment. The HR department and hiring manager/chief will be notified regarding the credentialing decision. You may not begin employment until all employment criteria has been met.
Important: If you are new to Kaiser Permanente, you may not treat Kaiser Permanente members and patients until you receive approval notification from the appropriate Credentials and Privileges Committee.
