Common issues that can delay the initial credentialing process

  • No license or expired license to practice in California

  • DEA does not reflect a California practice address or the DEA is reflecting an Exempt status.

  • Applicant submits incomplete or incorrect information on the application, for example:

    • Incomplete or missing fields or sections, such as Social Security Number, professional reference

    • 'Does not apply' boxes not checked where appropriate

    • Missing or improperly formatted date ranges (month and year are required)

    • Missing or unsigned Attestation and/or Release forms

    • Missing or incomplete explanations (e.g., professional practice time gaps, malpractice claims information, sanctions, limitations, disciplinary actions, criminal convictions, etc.)

  • Applicant does not submit a complete Credentialing application in a timely manner

  • Delayed responses from the applicant or external sources when contacted by credentialing staff for verification information or additional information

  • Contact information not current

  • Professional references do not meet criteria as defined in the application

How long does the initial credentialing process take?

The entire process, including privileging, can take 10 - 12 weeks, sometimes longer if staff experience delays as outlined above. This timeline begins the day a completed application is received by the Regional Credentialing Department and continues until the application and all supporting documentation is evaluated and acted upon by the applicable Credentialing and Privileges (C&P) committee(s) on behalf of Kaiser Foundation Health Plans, Inc., or Kaiser Foundation Hospitals. You will receive written notification when your application/appointment has been approved.

  • Practitioner submits incomplete or incorrect information on the application, for example:

    • 'Does Not Apply' boxes not checked where appropriate and Yes/No questions, including sub-questions, are left blank

    • Missing or unsigned Attestation and/or Release forms

    • Missing or incomplete explanations (e.g., malpractice claims information, sanctions, limitations, disciplinary actions, criminal convictions, etc.)

  • Practitioner does not submit a complete recredentialing application in a timely manner

  • Delayed responses from the practitioner or external sources when contacted by credentialing staff for verification information or additional information

  • Professional references do not meet criteria as defined in the application

Completing the Professional Liability Insurance section of the recredentialing application

Section 7A - Current Professional Liability Coverage of the application instructs the applicant to select one of two options:

  • I currently have no ($0) professional liability coverage.
  • I currently have professional liability coverage as indicated below.

Practitioners who select the first option must provide an explanation in the comments section.

Practitioners who select the second option must enter both insurance carrier contact information and policy information.

If you are an employed practitioner of Kaiser Permanente, you are afforded coverage through the Kaiser Permanente Medical Care Program. Please enter this in the "Name" field of this section. As specific policy details are currently on file, no other information is needed.
